  1. List of Board of Directors
  2. Chief Financial Officer
  3. Brief narrative of agency--purpose, mission statement, and program objectives ( ½ to ¾ page)
  4. List of all funding sources, including amounts of funding (If you prefer, submit instead a copy of your most recent audit)
  5. For currently funded agencies, a statistical report on referrals received for the budget year
  6. Program Agreement (ten copies)
  7. Final Program Agreement (two signed originals and five copies)
  8. Additional: Conflict of Interest Forms, Contracts and proof of Libality Insurance Information
Please note: Incomplete or late applications will not be considered for funding. Proposals are due in the United Way office by Friday, February 5, 2010 5:00 p.m. Incomplete or late proposals will not be considered for funding.
